Inspired by her years of volunteerism, community engagement, patient care, clinical research, and patient advocacy, Care Partners was founded in 2024 by Devika Nair, MD MSCI.

Connecting seniors, including Veterans, to youth experiencing unstable home environments to foster meaningful, bi-directional relationships
Youth Mentorship Support
Connecting youth who are primarily cared for by seniors with younger adults who can serve as mentors
Care Navigation for Seniors
Providing care navigation services for seniors, including but not limited to: assistance with advance care planning, communication with physicians, assistance with connecting with insurance agencies, assistance with interpreting health information.
